Get MCP connection config for an app

POST 
/integrations.v1.services.gateway.v1.IntegrationsGatewayService/PipedreamGetMCPConnectionConfig

Resolves runtime MCP connection configuration (base URL, access token, headers) for a given app and the current user. The access token is always freshly obtained at call time. external_user_id is derived from the JWT subject.
